Recount Ballot Box Report

This is the template of a recount ballot box report, page 1. The following information must be filled in: first, at the top of the form, the electoral district name, date of recount and ballot box number or special ballot types; second, under the heading “Initial recount by recount team,” the number of votes cast for each candidate, total valid votes cast, rejected ballots, spoiled ballots, unused ballots, disputed ballots and total ballots. Below, there is a section for unanimously agreed reclassifications, in which to record from which envelope to which other envelope a ballot was moved, and the reason for it.

For each ballot, the following information must be filled in: the ballot number (made up of the ballot box number and sequence number of the disputed ballot), whether the judge decided to reject or accept the ballot, in whose favour, and the judge's comments on the decision. The judge initials beside each line.

The following information must be filled in: the number of votes cast for each candidate, total valid votes cast, rejected ballots, spoiled ballots, unused ballots and total ballots. It is to be signed by the handler and the recorder, initialed by the candidates' representatives, and certified correct by the judge.
